Important Trial Terms
This form is used to request a 7-day evaluation of the Mortuary Balancing Bar™. Approved trial requests require a refundable $275 deposit and a $35 outbound shipping fee, which are collected separately after submission.
Trial Charges
A $275 refundable trial deposit is required to initiate the trial
A $35 outbound shipping fee is charged at checkout and is non-refundable
No additional charges are made during the 7-day evaluation period
How the Trial Works
One Mortuary Balancing Bar™ is shipped to your facility
The 7-day trial begins upon confirmed delivery
The bar may be used during real cases throughout the evaluation period
One trial unit per facility
Returns & Refunds
If the bar is returned within the 7-day trial window and according to the provided instructions:
The $275 trial deposit is fully refunded
A prepaid return shipping label is provided
The $35 outbound shipping fee is not refundable
End-of-Trial Outcome
If the bar is retained after the trial period:
The $275 deposit is applied toward the purchase price, or
An invoice is issued for the remaining balance
Additional units require purchase or invoice approval
